    What Internal Piles Mean

Internal piles are found inside the rectum and are not visible from the outside. They generally cause no pain or minimal pain, and rectal bleeding is the first sign of internal piles. When an internal pile comes out through the ans is called a prolapsed pile. This medical condition can be due to the wearing out of the muscles around the anys and can be very painful. Medical experts grade internal piles from 1 to 4depending on the degree of prolapse: grade 1 remains in the rectum; grade 2 prolapses when someone passes stool, then returns inside on its own; grade 3 are prolapsed and must be pushed back in; grade 4 piles are prolapsed and cannot be pushed back in.

In Ayurveda, internal piles are called Abhyantara Arsha, meaning piles that originate inside the rectum. The primary cause of piles in Ayurveda is Mandagin, weakened digestive fire, which leads to improper digestion, accumulation of Ama (toxins), chronic constipation (Vibandha), and straining during bowel movements. This strains the blood vessels and deranges the body's doshas: Vata, Pitta, and Kapha.

Who Is Affected And Risk Factors

Internal piles can happen to anyone at any age, including during pregnancy or early adulthood:

    Who Is Most Affected

    • People ageing between 45 and 65 years: People in this age group are usually affected by internal piles due to natural wear and weakening of anal cushion tissues.
    • Pregnant women: Internal piles also affect pregnant women because during this period the hormonal shifts relax vein walls and the growing uterus increases pelvic pressure.
    • Individuals with chronic bowel habits: People who are dealing with persistent constipation or chronic diarrhea face higher rates of symptoms in them.
    • Genetics: If someone in the family has piles, then the next generation may have a chance of getting it.

    Key Risk Factors:

    • Low fibre diets: People who eat low-fibre, refined foods tend to have harder stools that require more effort to pass.
    • Straining during bowel movement: Applying heavy pressure while having a bowel movement forces blood into the rectal vessels and causes them to swell.
    • Obesity: If a person has excess body weight, intra-abdominal pressure increases, which puts stress on pelvic and rectal blood vessels.
    • Heavy lifting: If you frequently lift heavy objects or weights in the gym, it raises internal abdominal pressure.
    • Other conditions: If a person has chronic cough, ascites, or pelvic/abdominl masses, then it can also elevate internal pressure.
    • Sedentary lifestyle: People who sit for prolonged hours put continuous gravitational pressure on lower rectal veins.

    Symptoms Or Presentation

Internal piles form deep inside your rectum. Because this specific area of your body has almost no pain-sensing nerves, you usually cannot feel them at all until something goes wrong.

    • Painless Bleeding: This is usually the first warning sign. Because these internal swollen veins are highly fragile, passing a hard stool can easily scrape them open. You might see bright red blood on the toilet paper, streaking your stool, or dripping directly into the bowl.
    • Bulging (Prolapse): If you push and strain hard on the toilet, the heavy pressure can stretch the internal vein and force it to bulge completely outside of your body.
    • Sudden Soreness: Internal piles only hurt when they are pushed outside. Once they drop out, your tight pelvic muscles pinch the swollen lump, instantly causing a dull ache, sharp soreness, and intense irritation.
    • A Heavy, Unfinished Feeling: Because these swollen lumps take up physical space, they constantly press against your internal nerves. This tricks your brain into feeling like you still need to go to the bathroom, even if you just went. You might also notice a sticky, slimy mucus when you wipe.

    How It Is Assessed/Diagnosed

A doctor does your medical examination by asking you about symptoms such as bright red blood on toilet paper or in the bowl, itching, a feeling of incomplete emptying, bowel habits, and family history as well. A bunch of tests such as Digital rectal exam, anoscopy, proctoscopy, and colonoscopy are done to view the lining of the anal canal and lower rectum to get the bigger picture.

An Ayurvedic practitioner does your Nadi pariksha (pulse diagnosis) to check which of the internal energies are out of balance in the body. He also asks you questions related to your diet, lifestyle, and digestion to get a better understanding. He also does your medical and physical examination.

    Management And Monitoring

Managing internal piles means strictly sticking to your new daily habits. Even after the bleeding completely stops, you must continue taking your prescribed herbs and eating soft, warm meals to keep your digestion strong.

Monitoring your condition is just as important. Pay close attention to your body in the bathroom. If you suddenly see new bright red blood, feel a sharp new pain, or notice a swollen lump that will not push back inside, tell your doctor immediately. Regular follow-ups with your Ayurvedic expert ensure your internal heat stays balanced, keeping the delicate veins under control before they flare up again.

    Ayurvedic Assessment And Supportive Care

Ayurvedic supportive care for internal piles focuses on balancing the digestive fire, internal energies, softening the stools, calming the inflammation, and healing the local tissues.

    Oral Formulation and Herbs

Ayurvedic herbs do more than just relieve constipation; they heal the root cause of the problem. Experts use specific herbs like triphala to naturally soften stool and gently clean your bowels every night. Other herbs cool your internal body heat and stop the bleeding and swollen internal veins to naturally shrink.

    Fixing Your Digestive Fire (Agni)

The very first step to healing is repairing your weak digestive fire. When your stomach easily breaks down food, you completely stop producing the hard, dry stools that constantly force you to strain and push in the bathroom.

    Clearing Sticky Waste Ama

Years of poor eating habits leave heavy, sticky toxins trapped deep in your gut. Specialized Panchakarma therapies actively flush this heavy waste out of your system, clearing the internal blockage that traps your downward-flowing body energy.

    Minimally invasive procedures

If large intestinal piles drop entirely out of the body, herbs alone may not work. Instead of painful surgery, kashara karma is used in Ayurveda. An expert applies a specialized alkaline paste or thread directly to the limb. This safely cuts off the blood supply, shrinking the swollen tissue naturally with minimal recovery time.

    Diet & Lifestyle Changes

You must completely stop eating dry, spicy, and deep-fried food that instantly triggers painful flare-ups. Switching to easy-to-digest meals with pure cow ghee and actively avoiding long hours of continuous sitting removes the heavy physical pressure on your lower body.

    Prevention Or Progression

If you ignore early warning signs of piles such as painless bleeding, internal piles will slowly progress. Over time, daily straining forces those hidden swollen veins completely outside your body, turning them into highly painful, permanent lumps that may require intensive procedures like Kshara karma.

To stop the progression, you must permanently fix your digestion. Drink warm water daily and add pure ghee to your meals to ensure your stool stays soft. Never ignore the natural urge to go, and stop bringing your phone to the toilet. If you completely stop the heavy pushing, your veins will stay safely hidden and pain-free.

Red Flags To Look Out For

There are certain symptoms that suggest a more serious condition or severe complication in an individual. These warnings must not be ignored and require medical care urgently:

    • If you experience heavy or continuous rectal bleeding.
    • If you notice that stool is dark, maroon, or mixed/ clooteed into the stool rather than bright red surface streaks.
    • If you have dark, tarry, or black stools.
    • If you experience severe or worsening pain.
    • If your internal piles push outside the anal opening and cannot be gently pushed back inside.
    • If you have unexplained and sudden weight loss.
    • If you have high fever, chills, severe swelling, or pus/ foul smelling discharged around the anal area.
    • If you have a feeling of a mass or persistent change in bowel habits.

    FAQs

They are swollen blood vessels located deep inside your lower rectum. Because they stay tucked away inside where there are few pain-sensing nerves, you usually cannot feel them in the early stages.

The lining deep inside has almost no pain receptors. When hard, dry stool passes through, it scrapes against fragile swollen veins. This causes drops of bright red blood in the toilet bowl without hurting.

Grade 1 stays hidden inside. Grade 2 drops out while pushing but slides back naturally. Grade 3 must be pushed back inside with your fingers, while Grade 4 remains trapped outside permanently.

Yes. Early stages heal completely with a soft diet, warm water, and no straining. Advanced stages can be treated using Ayurvedic Kshara Karma, a medicated alkaline therapy that shrinks lumps without invasive surgery.

Sitting for more than three minutes traps blood in your lower body. The steady downward pressure forces delicate internal veins to stretch and fill with blood, making early swelling much worse.

Ayurveda strengthens your core digestive fire to prevent hard stool from forming. Adding pure cow's ghee to warm meals lubricates your intestines, easing waste out effortlessly and permanently removing the downward pressure.
